Pilots and Flight Attendants Face Highest Radiation Cancer Risk


The people we trust to keep us safe at 30,000 feet face a quiet danger most passengers never think about. Pilots and flight attendants have the highest risk of dying from radiation-related cancers of any occupation in the United States, even higher than workers who handle radioactive materials every day.

I read the study with my coffee this morning and felt that small shift in my chest. The kind you get when something ordinary turns out to carry a cost you did not know about. Flying is routine for millions of Americans. We book tickets, board planes, and trust the crew. We do not think about what happens to their bodies up there, year after year.

A new analysis published in JAMA Internal Medicine looked at nearly 13 million death certificates from 2020 through 2024. The researchers examined more than 500 occupations across the country. They included about 14,000 pilots and 7,000 flight attendants in their review. The team was led by Dr. Anupam Jena, a professor of health care policy at Harvard Medical School.

What they found was clear. Flight attendants had about 50 percent higher odds of dying from a radiation-related cancer than the general working population. Pilots had about 36 percent higher odds. In terms of actual deaths, about 6.9 percent of flight attendant deaths came from these cancers. That is roughly 1 in 14. Among pilots, it was 6.7 percent.

These numbers stayed high even after the researchers adjusted for age, sex, race, ethnicity, education, and marital status. The risk did not disappear when they accounted for those factors. Cosmic radiation exposure at high altitudes appears to be the driver.

Why aircrew get more radiation than nuclear workers

It sounds strange at first. Nuclear technologists work with radioactive materials. They wear badges. They follow strict protocols. Yet aircrew members receive the largest annual dose of ionizing radiation of any workforce in America. The reason is simple physics. The higher you go, the thinner the atmosphere becomes. Earth’s atmosphere blocks much of the cosmic radiation that comes from space. At cruising altitude, that protection is much weaker.

The study found that flight attendants and pilots ranked first and second among all 503 occupations for the proportion of deaths from radiation-related cancers. Nuclear power plant workers ranked 12th. This proportion exceeded even that for nuclear technologists who are routinely exposed to radiation from non-cosmic sources.

The cancers linked to this exposure include breast cancer, prostate cancer, melanoma, and cancers of the central nervous system. Flight attendants faced 61 percent higher odds of dying from breast cancer and 75 percent higher odds for prostate cancer. Pilots showed 57 percent higher odds for breast cancer and 38 percent higher odds for prostate cancer. Melanoma risk was 63 percent higher for flight attendants and 28 percent higher for pilots.

I keep thinking about the person who checks your boarding pass, smiles, and shows you to your seat. They do this hundreds of times a month. They spend their working life at altitude. Most of us spend our working life in offices, stores, or warehouses. We worry about stress, or sitting too long, or the air quality in the building. We do not worry about cosmic rays.

The researchers say this is an occupational health concern that deserves attention. They called for consideration of stronger radiation protections for aircrew. That could mean better monitoring, more awareness, or changes to how flights are scheduled. It could mean treating cosmic radiation the way other industries treat known hazards.

What remains unknown is how this will change things. The study shows the risk clearly. It does not tell us what airlines or regulators will do next. The Federal Aviation Administration sets rules for flight time, rest, and safety. Radiation exposure is not currently regulated the same way as other workplace hazards in aviation. Some experts have argued it should be.

There is also the question of what passengers should make of this. A single flight does not pose meaningful risk to a traveler. The concern is for people who fly for a living, day after day, year after year. Their cumulative exposure adds up in ways ours does not.

I do not fly as much as I once did. When I do, I notice the crew more now. Not with pity. With a kind of quiet respect. They do a job that keeps the world moving. They also carry a risk most of us never see.

The study was published August 17 in JAMA Internal Medicine. It analyzed death certificate data that included occupation details. The researchers adjusted for multiple factors to isolate the effect of cosmic radiation exposure. Their findings support efforts to protect U.S. aircrews from this exposure.

About 12 million death certificates were reviewed in the broader analysis. The 50 percent and 36 percent figures represent adjusted odds compared to the general working population. These are not small differences. They are large enough to matter for policy, for health monitoring, and for the people who do this work.

What happens next is not clear. Studies like this one often take years to translate into rules or protections. In the meantime, pilots and flight attendants keep flying. They keep doing the job we all rely on. And they keep facing a risk that most of us will never have to think about in our own work.
